POSITION SUMMARY




The position is an integral part of the UKSM-W Medical Practice Association HIV Program. The primary function of this position is to assist people living with HIV/AIDS or those affected by HIV in obtaining resources and social service assistance including Ryan White programs, housing and patient assistance, dental services, transportation, and more. This position delivers HIV prevention education to clients as well as administers HIV rapid testing and counseling services.


This position carries a case load and but also assists clients that are not on their caseload. The Ryan White Part D Program is a state-wide program serving most of Kansas. Position is funded through a Health Resources and Services Administration (HRSA) grant whose focus is on women, infants, children, and youth (under the age of 25).


HRSA Ryan White Part D funding is intended to improve access to family-centered HIV medical care through the provision of coordinated, comprehensive, and culturally and linguistically competent services.


ESSENTIAL FUNCTIONS




Medical Case Management


Assessment of Client Needs


+ Assess health literacy, health risk, fiscal and housing stability, support systems and other social determinants of health for new, returning, and established women, infants, children, or youth


+ Identify any barriers to care, medication adherence and medical care adherence issues, and develop plans with clients to overcome barriers


+ Provides prevention and risk reduction education to clients, affected individuals and community members in one-on-one setting as well as at community outreach events.


+ Coordinate with healthcare providers to inform clients of research opportunities


+ Make home visits to clients and their families needing intensive case management, to better assess the home situation and their needs.


Connecting Clients with Services


+ Provide information on Ryan White programs (RW Parts B, C and D) and the Affordable Care Act (ACA) to eligible clients.


+ Assist clients with enrolling into federal, state and local programs which provide a network of support in the areas of primary healthcare including dental, mental health, and substance abuse treatment and other referrals, as well as supportive services (ie housing, transportation, food pantries and more)


+ Coordinate with Ryan White Part B, Part C, Linkage to Care, local and state-wide case managers to ensure clients continuity of care.


+ Document in the client case management data base and in their electronic medical records all interactions with clients in an accurate and timely manner.


+ Arrange supportive services (i.e. transportation assistance for medical visits, and emergency food and shelter and other services when needed.)


+ Assist clients with family planning, pre-conception counseling and chronic disease management. Report new pregnancies and births to their healthcare provider and other key staff as needed. Maintain/Update information in the National Antiretroviral Pregnancy Registry.


+ Must adhere to the Kansas Department of Health and Environments medical case management Standards of Care.


+ Works directly with WICY clients regardless of whether or not they are on the assigned caseload.


Program Development and Implementation


+ Perform HIV testing/counseling in both clinic and outreach settings.


+ Participate in developing educational materials and events, incentive programs, support grou ps or other activities that are targeted towards engaging WICY clients into care


+ Maintain the Wellness/Personal hygiene pantry


+ Attend state and local meetings as required or requested
