Supports all health care services throughout the residence.
Provides clinical services to Residents, train caregivers and supervises
direct care staff as required by DON/HWD.
Provides input for services planning for residents as well as renewal and maintenance of
Recommend Health Care Services goals and assist with their implementation. Work with the DON/HWD to supervise, oversee, and coordinate the work of caregivers to meet Residents’ needs.

+ Adhere to and convey a philosophy of supporting dignity, privacy, independence, choice and individuality for Residents.
+ Analyze, evaluate and report the needs of the Residents and report their needs appropriately.
+ Coordinate, in cooperation with the DON/HWD, ancillary health care and health education services for Residents.
+ Ensure all required documentation is completed accurately and thoroughly, to include service and medications delivery, and that all
+ records required by regulatory agencies or company policy are maintained effectively and according to guidelines.
+ Assists with sales activities to include touring, move-ins, Resident orientation, and move-outs as directed.
+ Assure infection control procedures are known and followed by all Associates. Track trends for Quality Management Review.
+ Attend care planning conferences and maintain service and treatment records for each Resident as required.
+ Responsible for identifying Resident problems, concerns or issues and providing appropriate follow-up to ensure resolution.
+ Conduct nursing assessments of Residents and provide for intervention as needed.
+ Evaluate health emergencies and ensure medical measures are taken to include delivery of first aid to Residents and Associates.
+ Act as Resident advocate in medical/social situations involving Resident and/or designated agents, physician or other agencies.
+ Supervision of direct care staff and monitoring the effective delivery of services to residents during assigned shift.
+ Notify the DON/HWD of any Resident status change as appropriate.
+ Review Resident service records and treatment and medication books to assure quality care.
+ Verify physician orders, move-in/move-out information and initiate appropriate action and follow-up.
+ Identify need for, develop, and conduct training as required by regulatory agencies and by company policy.
+ Night time LPN must be immediately available and awake.
+ Will keep certification current and participate in continuing education to include staff meetings, in-services and web based trainings.

Current license as an LPN, LVN (as required by state guidelines) to include annual continuing education requirements.
Current CPR and First Aid certification as required by state regulatory agency
Must be a licensed LPN, LVN (as required by state guidelines) and any necessary examination(s) in accordance with laws of this State. Prefer prior experience in AL or SNU.
Prefer prior experience as an LPN/LVN for at least 1 year.
+ Must possess a current, unencumbered, active license to practice as a Licensed Practical Nurse in this state.
+ Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
+ Must be able to read, write, speak, and understand the English language.

+ Must possess the ability to make independent decisions when circumstances warrant such action.
+ Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
