Integrated Care Consultant functions as a part of the primary treatment team. We are a medium-sized, fast-paced, multi-doctor practice located on the East of Wichita. Weekends Off!The Integrated Care Consultant provides assessments, identifies patient who would benefit from integrated care interventions, treatment plans to address and identify goals, evaluates crisis situations and intervenes appropriately, educates patients on prevention and treatment options, collaborates with primary care team to assist with integrated care of patients. ResponsibilitiesProvide assessment and diagnosis of behavioral health and chronic care patientsPlans treatment for patients to assist with achieving identified chronic care and behavioral health goalsIdentifies patients from patient population who would benefit from behavioral interventions.Provide behavioral interventionsConsults with medical providers, medical assistants, and nurses to assist with patient center careEducate patients , their families, and HHC staff on prevention and treatment of chronic conditions and behavioral health conditionsCreate process for follow up care for patients to assist with maintaining and managing their goalsIdentify, evaluate, and intervene with crisis situationsAttend clinic and other meetings as assignedMaintains clinical record in electronic health record and other required paperwork in timely mannerComplete SBIRT interventions and refer patients to appropriate services as necessaryCollaborate with HIV/AIDS outreach staff to assess, consult, and intervene with patients who are completing an HIV/STD testOther duties as assigned.QualificationsMasters degree in Psychology or Social Work, licensed in the state of Kansas, is required.Licensed Specialist Clinical Social Worker (LSCSW) preferred.Minimum of two years' experience in behavioral health services preferredExperience with integrated care preferredA bilingual individual is preferred, but not required.
