Provide comprehensive healthcare management to facilitate delivery of appropriate quality healthcare, promote cost effective outcomes and improve program/operational efficiency involving clinical issues.

Fundamental Components:

The Continuum of Care Manager is accountable to the PM Director for participating in the development, implementation and ongoing monitoring of program initiatives to address the needs of Aetna members who are receiving services along the continuum of care. The Manager's focus is on early identification of members who need services, coordination of care management services among multiple Aetna program areas, integration of program initiatives within the Care Teams and professional development of staff. The Continuum of Care Manager will also serve as the single point of contact for Clinical Customer Service Managers, will assign cases referred by the Clinical Customer Service Manager to the appropriate Care Team and will respond to issues and inquiries as needed. The Continuum of Care Manager collaborates with regional Medical Directors, Care Management Managers and Care Team Supervisors to enhance the quality of services provided to Aetna members and to ensure adherence to performance targets developed by National Patient Management (NPM) or the region.


3-5 years experience as a Case Manager, Aetna experience as a Case Manager desired.

Case Management Certification desired.

3-5 years experience supervising Case Managers, Aetna experience as a supervisor desired.

1-2 years experience in other care management programs.

5-10 years experience in a clinical setting; required


The highest level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.


Registered Nurse RN; Required
