Service Provision Evaluation Services and Therapy.Complete client intake/assessment to include history, presenting problems and diagnoses using DSM-V criteria.Individual and group therapy using identified and accepted methods to remediate symptoms of mental illness and/or substance use treatment to improve emotional and functional well-being as needed.Conduct risk assessments as needed to address safety of clients.Demonstrate flexibility in adapting to workflow when completing scheduled and unscheduled work assignments.Initiates outreach to increase engagement.Treatment Planning.Prepare quality treatment plans in collaboration with client and/or guardian.Facilitate treatment plan review, addressing updates and progress on goals with client's treatment team if applicable.Documentation.Complete intake summaries, risk assessments and agency referral documentation within 48 hours of meeting with a client to complete an intake for services.Complete Notes for Record, precautions, authorization for release of records, releases of information, safety documents and other documentation as required and/or recommended within established deadlines.Enters intake cancellations for accuracy and completeness of medical record. Minimum Qualifications: Master's Degree or higher. Must be licensed by the Behavioral Sciences Regulatory Board as an LMSW, LMFT, LMLP, LPC, or LP.
