The Medical Assistant supports the Care Management Team medical providers and ensures that all patients receive quality care while maintaining compliance with all federal grants, HIPAA, and Hunter Health Clinic policies and procedures.


Care Management Responsibilities:

+ Assists in implementing case management programs.

+ Partners with providers and care teams to complete designated office visits.

+ Prepares reports by collecting, analyzing, and summarizing results data and trends, compiling statistics.

+ Collaborates with the care teams.

+ Performs daily monitoring, recording and data reporting of patients’ information received by monitoring systems (i.e. blood pressure, blood glucose, weight, etc.)

+ As requested, will assist in coordinating services such as arranging resources, including transportation and escort.

+ Maintains confidentiality of patient and proprietary information.

+ Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.

Medical Assistant Responsibilities:

+ Assists Physicians, Mid-Levels, RN’s, and LPN’s with patient care, as requested.

+ Prepares for patient visits to include identifying needs, setbacks, orders; lead daily care team huddle with completed visit prep; shepherd and maintain effective workflow; room and exam preparation; rooming of the patient.

+ Obtains the patient’s temperature, pulse, respirations, blood pressure, height, weight, head circumference (infant/pediatric), and records values in EHR.

+ Utilizes data tracking boards to identify setbacks and opportunities for improvement at the end of each session.

+ Documents Chief Complaint in EHR.

+ Performs the following: Peak Flows, nebulizer treatments, Pulse-Oximetry, 12-lead EKG and Snellen test, capillary blood glucose test, A1C, and INR, strep test swabs, TB skin tests (ID injection), IM injections, and SC injections, Electrocardiography, suture and staple removal.

+ Performs medication administration to include withdrawing medication from the vial/ampule, adhering to the rights of med admin (right patient, right medication, right dose, right route, and right time), withdrawing medication from vial and an ampule, and proper oral administration (liquid or pill).

+ Assures the entry of medications in ‘outpatient medications’ within the MAR in the EHR.

+ Accurately reads and documents the TB skin test.

+ Enters current and historical childhood and adolescent immunization in Web-IZ and EHR.

+ Cites vaccinations, administration sites, and recommended ages for the vaccines.

+ Enters Lab orders, within the HER, with the proper completion of lab requisition forms.

+ Collects and documents (EHR) the Patient History prior to the patient being s een by medical provider.

+ Stocks patient rooms, storage areas, work areas, biohazard disposal, sharps containers, and updates the infection control log.

+ Prepares the following: exam rooms for assessments (stocking, disinfecting, cleaning of equipment and surfaces); pap smear and STD test set up; sterile tray set-up; patient preparation prior to the medical provider’s arrival.

+ Performs septic handwashing before and after rooming patients in exam room and procedures.

+ Proper dismissal of patient to include education.

+ Assists RN/LPN with telephone encounters documented daily in EHR.

+ Documents patient encounters, phone calls, drug administration, and chart review in EHR.

+ Assists RN/LPN with prescription refill requests, daily. All prescription refill requests must be reviewed and approved by the medical provider and/or nurse and faxed same-day refill request received.

+ Accurately completes all forms such as Gynecological (Pap Smears/STD) work-up, EDW forms, and pharmacy forms.

+ Assists in inventory and keeping storage supply closet neat and free of debris.

+ Performs autoclave operations: Completes weekly and monthly maintenance for autoclave and hydro-sonic sterilization of instruments, performs the ATTEST, and updates the manual after completion.
