Minds Matter, LLC Intake Coordinator in Overland Park, Kansas

Job Summary

This position will require knowledge of all services and resources provided by our company and within our community. The Intake Coordinator must have vast knowledge of social security, Medicaid, Medicare, private insurance, and other funding streams. In addition, the intake coordinator will serve as the central point of contact for quality assurance, written and oral communication internally and externally on all referral processes. This position requires frequent travel in Johnson County, Wyandotte County, and surrounding areas. The intake process takes place in the community and in consumers’ homes.

Responsibilities and Duties

  • Responsible for learning and implementing all aspects of Minds Matter LLC philosophical principles.

  • Facilitate the ongoing eligibility of consumers for services including medical eligibility, knowledge of private insurance benefits, Social Security, Medicaid, Medicare, Kansas Brain Injury waiver, and other funding options.

  • Conduct intake process within timelines outlined by tracking sheet.

  • Maintaining database for consumer information, referral contacts and service outcomes, maintain list for Aging and Disability Resource Center (ADRC) referrals and contact periodically to follow up on progress with connection to needed services and disseminating new referral information to the Vice President of Outreach, Director of Rehabilitation Services (DORS), Regional Rehabilitation Directors (RRDs) and other staff, as appropriate.

  • Answer telephones and give information to callers, take messages, or transfer calls to appropriate individuals.

  • Show proficiency in technical aspects of the waiver including, but not limited to, electronic plans of care, documentation, paperwork, reviews, and forms for communication between consumers, Kansas Department for Aging and Disability Services (KDADS), Managed Care Organizations (MCO’s) and all other agencies.

  • Communicate all known needs and service desires of consumers to the Vice President of Outreach, DOR, RRDs, and other staff, as needed.

  • Meet all policy and procedure requirements for documentation, including completion of reports.

  • Make copies of correspondence and other printed material.

  • Establish and maintain a professional rapport with all consumers, co-workers, referral agents and representatives of other community agencies and services.

  • Promote public relations throughout the agency and community for the program.

  • Demonstrate a willingness to serve on agency and inter-agency task groups as requested.

  • Conduct services in a professional manner including abiding by Minds Matter’s dress code. The dress code states that if staff is meeting with new consumer/families or interacting with other professionals they are not allowed to wear jeans, shorts, sweat suits, tank tops, flip flops, or other “casual wear”. Professional staff will be allowed to wear jeans on Fridays if they are not meeting with new consumers or families.

  • Operate office equipment such as fax machines, copiers, and phone systems, and use computers for spreadsheets, word processing, database management, and other applications.

  • Perform other duties as assigned.

  • Maintain confidentiality of protected health information in accordance with HIPAA regulations.

    Qualifications and Skills

  • High degree of sensitivity toward all people

  • Ability to effectively plan, coordinate and assist with intake process and systems

  • Ability to make evaluation based on measurable criteria

  • Ability to interact professionally with consumers, staff, and professional contacts

  • Customer Service Skills and a high degree of professionalism

  • Computer skills including experience with Excel and Power Point

  • Excellent time management skills, often changing from one task to another without loss of efficiency or composure

  • Excellent Oral and written communication skills

  • Strong attention to detail

  • Basic to moderate technology skills, as our electronic medical record system is run on an iPad.  

    Education and Experience

  • Prefer bachelor’s degree in social services, business and/or 1-3 years of related experience.

  • Training in Microsoft products and other applications

  • Prefer prior experience working with persons with disabilities and systems.

    Requirements

  • Requires good verbal skills and listening ability.

  • Valid Driver License

  • Must be able to read and write

  • Basic iPad, iPhone and Laptop proficiency

  • Have reliable transportation

  • Check voicemail, text message and email daily.

    Measurement of Success

  • Feedback by constituents including consumers, families, staff, and other agencies.

  • Program utilization by referral agents.

  • Ability to relate to and work with all consumers effectively.

  • Degree of confidence the Vice President of Outreach has in the execution of responsibilities.

  • Positive and professional relationships with all internal and external contacts.
