The incumbent provides therapeutic groups, individual therapy, treatment planning and assessments for the patients admitted to the hospital. The incumbent is identified as the primary therapist that assists the patient throughout their hospital stay with any situations related to their psychiatric care and treatment. This includes routine understanding of the therapy process, crisis intervention, assistance in post-discharge support to reduce re-hospitalization, revisions to the treatment plan and working across disciplines or with the Treatment Team.Job Responsibilities may include but are not limited to the following: Psychotherapy: Provide group and individual psychotherapy to reduce symptoms/distress, educate the patient, and facilitate placement in the community. Provide crisis intervention. In designing and providing therapies, the incumbent is mindful of how psychotherapy complements other therapies and the hospital's philosophy of short-term treatment. Documentation of these services is meaningful and timely.Treatment Planning, Assessments & Evaluation: For assigned patients, treatment plans are prepared in conjunction with the input of the patient, other team members and are based on the presenting clinical symptoms. Treatment plans are updated as needed throughout the patient's hospitalization. Risk assessments are completed within assigned time frames as indicated by patients presenting problems and course of hospitalization. Other general assessments are completed as requested by the Interdisciplinary Team (IDT). Monitor the effectiveness of the interventions included with the treatment plan. Document treatment plans and assessments in a clear, complete and timely manner. With supervision as needed, clinical planning and diagnostic considerations are included during the initial treatment planning period and throughout the hospitalization. Within the scope of licensure and with supervision as appropriate, complete formal evaluations using accepted professional standards and according to hospital timelines. Findings are communicated in a timely manner through staff conferences, direct feedback to patients, court testimony (when required), and written reports.Teamwork & Supervision: Coordinate patient treatment plans with the IDT and the other members of Psychological and Therapy Services (PTS). When assigned as team leader, provide leadership to the IDT and shared leadership of the PTS staff assigned to the treatment team. When assigned, facilitate meetings with the PTS and attend program management meetings as the PTS liaison. Work with the IDT to educate patients regarding psychiatric disorders, treatment, diet & fitness and self-care. Document the results of patient meetings, team meetings and the input of other staff as appropriate, according to hospital policy & procedures. Attend and participate in service and program meetings. Provide leadership in defining and maintaining the therapeutic environment on the incumbent's ward / program. Provide supervision of students as assigned. Provide consultation and training to professional and direct care staff in order to improve treatment services. Participate in program development and evaluation.
