PERCENTAGE OF TIME ACCOUNTABILITIES90% Oversees the Pregnancy Maintenance Initiative Targeted Case Management and Teen Pregnancy Targeted Case Management10% Provide Social Work counseling to the Health Department clients upon referral and Maternal and Infant Program.SCOPEServes as the Health Department Social Worker under the general supervision of the Health Department Director of Nursing. Plans, organizes, and directs all aspects of Pregnancy Maintenance Initiative Targeted Case Management and Teen Pregnancy Targeted Case Management, and per Grant requirements. Will provide Social Work services to clients enrolled in the Health Department Maternal and Infant Program. Performs other Social Work duties as assigned.KNOWLEDGE AND SKILLS: (*Required prior to employment)• Knowledge of cultural competency in working with a diverse group of clients.• Knowledge of various components of the HIV/AIDS disease• Skills in data entry• Skill in making public presentations in the community• Knowledge of the principles and practices of social work and perceptive needs of people in crisis• Knowledge of available Social Services in the area; local, state, and federal and how they are accessedHUMAN RELATIONSWorks with local and state agencies in providing services to enhance the quality of life for residents of Leavenworth County. Works closely with multiple social service agencies with collaborating services for prevention and care in the HIV/AIDS field.ILLUSTRATIVE TASKS (This is not an inclusive list, other tasks/duties may be assigned.)• Attends case management meetings and programs with collaborating agencies• Directs all aspects of Pregnancy Maintenance Initiative Targeted Case Management and Teen Pregnancy Targeted Case Management• Meets with all clients in the Health Department Pre-Natal Program. Provide assessments, counseling services, stress management and social services referrals.• Obtain Child Passenger Safety Technician certification from the Kansas Traffic Safety Resource Office.ENVIRONMENTAL DEMANDSWork is performed in an office setting; travels by auto to site locations and meetings; walks over varying terrain in various weather conditions.MINIMUM QUALIFICATIONSMust have a Bachelor Degree in Social Sciences and licensed Social Worker in the state of Kansas. Possession and retention of a valid Kansas driver’s license.ADA/PHYSICAL DEMANDS1. Vision. Adequate for preparing and reviewing casemanagements, clients’ charts, other paperwork.2. Hearing. Adequate for conversation and telephone use. Considerablepublic contact.3. Speech. Must speak English clearly enough to give instructions toemployees and converse with public.4.
