HaysMed Sleep and Diagnostic Center Patient Care Coordinator in Larned, Kansas
Performs clerical duties independently, including but not limited to answering phones, schedules appointments, maintains fax queue, correspondence, filing, copying, scanning, receiving and distributing mail, maintains calendars and meeting schedules. Completes registration process, collects patient identification and demographics.
Schedule trainings.
Prepares, maintains and files/scans records.
Maintains fax queue.
Receives/distributes mail, answers phones, screens and directs callers.
Retrieves records from storage.
Maintains reception/waiting area.
Support activities, reporting to Office Coordinator.
Enters treatment completion information into databases.
Retrieves information for statistical purposes.
Ensures integrity of data.
Assists with public relations, vendors, outside consultants, etc.
Completes and maintains registration process.
Collects patient identification and demographics, HIPAA forms, insurance information.
Coordinate telemedicine outreach clinics, including technology checks.
Coordinate information necessary for outreach clinic visit, billing and record management.
Coordinates scheduling and agenda preparation of meetings.
Serves as note taker and prepares and distributes minutes.
Documents indicated signs/symptoms and consent for treatment.
Schedules appointments, tests and procedures.
Maintains current knowledge of ICD-10 and CPT codes.
Sends patient communication regarding appointments/tests/procedures.
Follows an individualized plan of care that promotes positive immediate and long-term outcomes for the patient/family and reflects the plan of the entire health care team.
Carries out interventions safely and in compliance with policy to facilitate achievement of expected outcomes as delegated.
Accurately documents patient response to intervention/therapy based upon expected outcomes.
Carries out delegated patient care activities and communicates with other members of the healthcare team as evidenced by positive patient outcomes.

Infection Control: Initial and Ongoing training in dealing with infection control. Trainings could include but are not limited to, blood borne pathogens, bodily fluids and bio hazardous materials as it applies to your daily work environment.
HIPAA: This position will have access to the following Protected Health Information in order to carry out the duties related to their position at Pawnee Valley Community Hospital based on the following criteria:
Primary - required (routine) to do the job
Secondary - required for the job, but mostly be exception
None - no approved access

Primary: Patient Demographic Information (information used to identify a person): Name, Date of Birth, Address, Race, Marital Status, Religion None: Financial Information/Insurance (information related to insurance, billing and payment): Billing Information, Payer Name, Payer ID, Account Balances, Plan Elements Covered, Payment Information, Payment Rates Clinical Information (information that describes a patient's health status): Diagnosis, Reports/Medical Notes, Test Results, Problem List, Procedures, History and Physical Coding Information (clinical information that is in (alpha) numeric format): ICD-10 Codes, Rev Codes, CPT Codes Infection Control:Initial and Ongoing training in dealing with infection control. Training's could include but are not limited to, blood borne pathogens, bodily fluids and bio hazardous materials as it applies to your daily work environment.
Required:
Excellent communication skills; ability to multitask; organization skills; basic computer skills.
BLS certification required or must complete BLS certification with in the first two weeks of employment.
