general responsibilities: Ability to implement a centralized intake and referral system and protocol that meet the requirements, plans, and agreements of the Kansas and Montgomery County Maternal, Infant and Early Childhood Home Visiting Program. Liaison and coordinate with community services in Montgomery and Wilson County including home visiting programs and other early childhood development, social, education and health service providers.

essential job functions:

  1. Quantity: Manage all aspects of the position. Develop and implement plans for a centralized intake and maintain a referral system. Collect data; generate reports and information about the home visitation outcomes and activities.
  2. 2. Clinical: Provide assessments for families to determine what services best fit their needs. Regularly assess the community home visitation needs and facilitate planning with the community collaborative and partners.
  3. Judgment: Demonstrate sound judgment that is in keeping with ethical and practice guidelines. Model good self-management skills and appropriate professional boundaries consistent with this agencys values.
  4. Coordination: Perform outreach and marketing strategies that find and engage families, perform family assessment and intake processes, and determine the best fit for referral. Coordinate between the involved Maternal, Infant and Early Childhood Home Visiting programs and other available resources. Coordinate cross program training and parent education groups.
  5. Documentation: Manage a data system/record keeping system that is current and up to date with all information needed for the program contract Provide documentation that is required for the contract and the coordination with the other home visiting programs. Assure accuracy of records and entered data.
  6. Leadership: Model respectful, supportive and professional conduct in all work environments. Promote community awareness and acceptance of the home visiting programs and centralized intake and referral system. Present and report information on the centralized system protocol, activities, and service provision data to community partners and stakeholders.
  7. Collaboration: Attend community interagency meetings as needed to facilitate services for families with at-risk pregnant women and young children (birth to five years of age).
