POSITION DESCRIPTION
POSITION TITLE: Charge Nurse - LPN JOB CODE: TBD
REPORTS TO: Director of Nursing Services STATUS: TBD
DIRECT REPORTS:

ACCOUNTABILITY OBJECTIVE:
Provides nursing care and oversight of direct care givers to meet the needs of
patients/residents
BACKGROUND/QUALIFICATIONS:
1. Current license by the State Board of Nursing as a Licensed Practical Nurse/Licensed
Vocational Nurse with experience in the long-term care industry preferred
2. Ability to plan, supervise and coordinates services.
3. A preference for contacts with people and patient centered care
KEY RESPONSIBILITIES:
1. Receives report on patient/resident status from previous shift and provides this
communication to oncoming shift
2. Makes physical rounds in assigned area to evaluate care and interact with care givers and
patients/residents
3. Provides direct care in line with the responsibilities of a nurse, evaluating conditions,
communicating and carrying out nursing functions according to the needs of the patients
4. Assists in establishing a plan of care for patients based on their unique needs and in line
with physician orders
5. Is a member of the Interdisciplinary Care Team
6. Accurately documents patient/resident condition including evaluations, communication
and care provided
7. May provide direct care including medication pass, nursing care, documentation and
treatments
8. Evaluates performance of nursing assistants and trained medication aides; coaches and
disciplines as needed.
9. May responds and coordinates actions in emergency situations
10. Other duties as assigned by Director of Nursing
11. Assists with the Continuous Quality Improvement initiatives
12. Ensures safe work practices are followed
13. Provides empathetic communication to families, patient/residents regarding change of
condition and in response to care concerns, ensures resident rights are respected

Lifting of 50 lbs. maximum with frequent lifting and/or carrying of objects weighing up
to 25 lbs.; walking and standing most of time on duty; frequent reaching, handling, and
fingering of instruments, equipment, records and reports
Physical Environment
Clerical perception is necessary to prepare records and charts and to organize training
programs; a preference for scientific and technical activities for understanding and
responding to medical problems and concepts; ability to interpret operating policies
and procedures and to review work performance in determining conformance to these
recognized standards; is around bio- hazardous waste and blood-borne pathogens.
Sensory Attention
Talking and hearing essential to instruct and supervise nursing personnel; near- visual
acuity required to detect changes in conditions of residents; color vision
